The information shown in the tables on this page is summarized from injury and illness reports submitted to PMA in 2012. The lost-time injury and illness incidence rate is based on Occupational Safety and Health Act (OSHA) record-keeping criteria and is a national standard used by the government and most industries to provide an overall indication of injury and illness trends. The formula for the lost-time injury and illness incidence rate includes the number of lost-time injuries and illnesses that occurred in the workplace and the total hours worked during the period (usually one year).It is based upon a work force of 100, each working 2,000 hours per year.(Number of injuries and illnesses x 200,000 total hours worked = Incidence Rate) Year Coast Northern California Pacific Northwest Oregon Washington 4,0074,007 6,67510,682 5,46416,146 1995 10.90 8.90 15.6011.50 1996 GST IV Going Home Safe 2000 2001 2002 Southern California 12.80 10.40 9.30 14.3012.70 9.90 1997 9.40 8.20 11.6011.20 11.20 1998 9.20 6.80 15.1013.90 12.40 1999 8.67 6.64 13.7012.60 11.20 2000 7.20 5.68 9.8110.70 10.70 GST V Aware Today, Everyday 2001 8.40 6.60 13.309.64 12.60 2003 2004 2005 2006 2002 8.50 6.49 14.1011.20 13.30 2003 7.50 6.00 10.5010.00 11.90 2004 6.77 5.71 9.049.95 9.11 2005 7.12 6.15 9.379.19 9.06 GST VI Every Choice Counts 2006 6.41 5.13 10.696.79 9.32 2007 2008 2009 2007 5.92 4.67 10.906.34 8.06 2008 5.92 5.00 9.497.38 6.81 2009 7.57 6.73 10.638.09 8.59 GST 2010 5.81 4.96 8.327.56 6.78 2010 2011 2012 2011 5.43 4.57 7.528.11 6.02 2012 5.30 4.44 7.98 9.08 5.23 3,4433,443 9,73313,176 12,33225,508 6,96632,474 10,70410,704 8,52319,227 5,38824,615 8,5938,593 7,57216,165 10,74626,911 ACCIDENT PREVENTION TOP TENS FOR 2012 Most Injured Occupations Most Common Injuries Most Injured Body Part Lasher268 Semi-Tractor258 Mechanic195 Holdman 177 Foreman/Walking Boss 102 Dockman86 Clerk Supervisor 70 Auto Driver 61 Crane Cont Gantry 49 Clerk Computer 45 30 Cause of Most Injuries Strained423 Struck by 203 Slip/Trip/Fall <4ft 164 Struck Again 133 Slip91 Noise Long Term 78 Unknown 59 Bounced in Vehicle 58 Twisted52 Pinched48 Sprain/Strain/Spasm665 Multiple Type 449 Contusion278 Cut, Laceration 120 Hearing Impairment Illness 78 Unclassified/Undetermined48 Foreign Object in Eye 42 Fracture20 Puncture14 Other Illness 11 Back216 Knee158 Fingers130 Shoulder106 Ear Internal 80 Head66 Ankle66 Hand61 Eye55 Foot36 2012 ANNUAL REPORT
